Ankle sprain: why a gradual return matters

After an ankle sprain, less swelling alone does not determine readiness to return. Understand the injury, then consider movement, strength, balance and the demands of your activity.

After an ankle sprain, less swelling alone does not determine readiness to return. Understand the injury, then consider movement, strength, balance and the demands of your activity.

First, understand the injury

A sprain involves ligaments, but twisting the foot can cause other injuries. Not every swollen ankle is a minor sprain. Difficulty bearing weight, severe pain or deformity needs appropriate assessment before attempting exercises or returning to play.

Protection is not endless inactivity

Protection and reduced activity may be needed initially. Movement and weight bearing then depend on injury severity and professional advice. A friend’s programme does not automatically suit you, and a photograph of swelling cannot determine rehabilitation needs.

Rebuilding movement and control

Assessment may guide work on ankle movement, strength and balance. Walking straight is different from descending stairs or changing direction. Tasks progress according to pain, swelling and function, without prescribing the same repetition count for everyone.

Return is more than a date

Less pain is encouraging but does not alone prove readiness to run or jump. Discuss activity demands, control and confidence. Renewed swelling or marked difficulty after increasing load provides information for adjusting progression rather than ignoring symptoms.

When to seek help

Seek assessment if symptoms worsen, persist or prevent walking. Deformity, numbness, a cold foot or abnormal colour requires urgent care. After diagnosis, injury or sports rehabilitation can be discussed at PhysioFlex according to your goals.

Walking is not changing direction

Level walking may become comfortable while uneven ground or sideways movement remains difficult. Describe work and sport demands at assessment. Completing one task does not prove readiness for all tasks; progression should relate to the function you need.

Observe after activity as well

Swelling may appear after a period of walking or training. Record the activity and symptom changes instead of increasing load to make up for missed days. Review progression if instability recurs or small increases repeatedly bring swelling back.

Your questions, answered

Can I massage the ankle immediately?

Do not massage an unassessed acute injury. Ask whether and when it is appropriate for your situation.
